Change ‘add a review’ title and link to review in review reminder

Hello,
I am just looking over the review reminder email and when you click it, it takes you to the top of the page with reviews on it, but there is no obvious way to leave a review. At the bottom of the page, there is no title on the Leave a Review heading, so people don’t know easily what they are reviewing.
