Assistance Needed: Creating a Review Form for Store Feedback

Dear Support Team,

I am currently trying to create a dedicated page on my website for customer reviews about our store. However, I have encountered an issue where the review submission form only displays for a specific product. I would like to have a form that allows customers to leave general feedback about the store itself, not tied to any specific product.

Additionally, I’ve noticed a layout issue with the star rating system. The stars are displayed across two rows, with the fifth star appearing in a separate row below the others (as shown in the attached screenshot).

Could you please assist me in resolving these issues? I would greatly appreciate your guidance on how to set up the page correctly and fix the layout problem.
