Change Fields in Form

Hi there!

I’m setting up the photo reviews right now and have an issue with the review form. I don’t want my customers to receive an reminder email about the review, but still I want to offer the coupon code to them. For that they need to leave there name and email address, but there is no field to fill this information in. I sent you a screenshot of how it looks for me right not (my version.jpg) and how I would like it to look. I know that the owner of the example screenshot that shows how I would like it to look like uses Photo Reviews for Woocommerce as well. Is there any option to change the fields and what it says in and above the fields? So that I can add a name and email option but then of course hide the email for public?

Maybe I missed this option in your plugin but I couldn’t find it anywhere.
