Customer Submit A Review Page Shortcode Not Working

Hi, I have added the shortcode to a new page I created to add the Review form for customers to fill out. I also selected this new page in the review settings. However this page is not showing the review form at all 🙁 I also tried editing the page and looking for the “Photo Reviews” item to add to the page and it is not there / an option to add.
