Design the couponbox that shown on shortcode

Hi,
I designed the Popup subscription form and I wanted to show also in a form on some pages.
When I used the shortcode: [wcb_widget always_visible=1 show_coupon_code=1]
You can see that I get a very poor designed form with just email field and a button.

I have two questions:
1. How can I edit the form fields and design on the shortcode
2. Is it possible to build few different forms with different coupons (5% off, 10% off , etc)?
