js errors on form page

On custom form page (which uses your plugin shortcode), I’m getting js errors relating to plugin “Woocommerce Cache Handler”

This plugin doesn’t seem to mix well with your plugin. On normal product page, there are no conflicts. Issue only seems to be on page with your review form.

I even tried to block Woocommerce Cache Handler from loading on this form page using “Asset CleanUp” plugin, but it appears you plugin is pulling this js script from the original product page.

Any suggestions on how to resolve these js issues?
