js errors on form page2

I’m not sure if you can help, but we have one other issue. If you go to form page now, you will see the form is missing:

We use WordPress LiteSpeed Cache, and we need to use the ESI feature of this plugin:

We use ESI for other parts of our website, but for some reason, when ESI is enabled, it’s blocking the form from displaying.

I didn’t have ESI turned on when you visted/worked on form previously, but now ESI is enabled. This is why form is missing. I’ve tried to exclude this custom form page in LiteSpeed cache, and numerous other workarounds, but I’ve had no luck in getting form to show (when ESI is enabled).

Any suggestions on how to have form visible/working when ESI is enabled?
