My review form its not working

Hello, I have an issue with the review form in my page, I use the shortcode [woocommerce_photo_reviews_form product_id=”” hide_product_details=”” hide_product_price=”” type=”popup” button_position=”left”] but it doesn’t work, when I clic the button “Write a Review” it shows as blank page, I need to resolve this.
