Order bump doesn’t show up on checkout

I just installed the plugin and setup an order bump but it doesn’t show up on my checkout page. Can you help me with this? I use the Breakdance theme, but when I switch to the standaard twenty-three it doesn’t work either.

It is a development site, so it’s not a live customer website.
