Page not found after review submit!

Hello, I have an issue where I cannot submit review as after I click “submit now” it breaks CSS of the website and throws “Page not found” error, it looks like it’s been happening a lot as I noticed no customer left review for the last few weeks. I cannot provide WP access, could you please tell me what to check and I will provide you with the update?
