shortcode does not show form

The description shows that I can add a shortcode [woocommerce_photo_reviews_form] to allow people to write a review on any page (not just in the tab on the product page, but I can’t get this to display a form. The reviews are there in the product page, and I can show the actual reviews on another page, but not the form to write one.
