Shortcode don’t display other fields

    Hi,
    I use the plugin for subscribe the users with popup mode and shortcode in specific pages.
    In the popup mode we set a birthday and name and surname fields and those are display correctly.
    We would like to repeat the same structure of fields in the shortcode visible in this page: https://stagingande.it/
