The field that allows visitor to enter amount doesn’t appear anymore

On a flooring product page, which has a cross sell item, there is no longer a way for a visitor to enter in the amount of square feet. It only shows the number of boxes below the cross sell item. I need for the visitor to be able to enter the amount before, the cross sell item, on the product page, before he or she clicks the add to cart button.

Below is a page in which I did not include a cross sell item. You’ll notice that there is a field for a visitor to add the sq ft. I need it on the page which also has a cross sell item.
