Woocommerce photo reviews review form now orking.

Hello there I have purchased the woocommerce photo reviews plugin the reviews are showing up but review form is not working. I tried placing the form using both the methods, the form and the shortcode but none of them actually work. Please this is very urgent and I need to get this fixed ASAP.
Also, yes ratings are turned on in woocommerce
