Multiple Issues With Wheel

I have the following problems while I am trying to implement the wheel:
-Having added the email field to the form, it now shows an email as the default for the input as a placeholder, which is not appropriate.
-The GDPR button does not show a tick when it is clicked
-The emails do not send after a wheel has been spun, I have used both Mailchimp and Sendgrid API keys, neither of them seem to work.
