I just noticed that the plug-in is not sending the correct e-mail for refunded orders, or partially refunded orders – customers are receiving the standard e-mail from WooCommerce, not the one from the plug-in.
I just did a test by actually refunding an order and confirmed it is not sending the correct one, both for full or partial refunds. All other e-mails are working fine, where it is sending the one with layout from the plug-in.
I double checked that the E-mail type setting is set correctly to Refunded order (full) / (partial) on the page for the e-mail in your plug-in.
