Single Page Order Review Issue

Hi. I created a single page order review.

I have put [woocommerce_photo_reviews_form] shortcode on the page.

But when customers click RATE button on My Account page, it redirects customers to XXXX.com/review_order/?order_id=XXX

And there is noting in the page body. It is just empty (there is no error in console either. Attached screenshot)

I even updated to the modified plugin file, you mentioned at this topic link below:
